Acne medications and treatments sold over the counter have varying formulas. There are some main ingredients that are more commonly used than others. While prescription formulas have a form of antibiotic included, treatments on sale for over-the-counter (OTC) use generally do not.

Benzoyl Peroxide

Benzoyl peroxide is a common ingredient in anti-acne creams and medications. It works by reducing the volume of pimples in a breakout, both whiteheads and blackheads. It is recognized by the U.S. Food and Drug Administration (FDA) as being generally recognized as safe (GRAS). Cosmeticsinfo.org reports that it may be called dibenzoyl peroxide and consists of two benzoyl groups. These groups are the peroxide group and benzaldehyde with the hydrogen of the CHO removed.

Salicyclic Acid

Also referred to as beta hydroxy acid (BHA), salicyclic acid is from willow tree bark. The salt and ester of salicyclic acid are used in many cleansing and skin care products. The FDA permits these as indirect food additives. Salicyclic acid is listed as an anti-acne agent, exfoliant, skin conditioning agent and preservative. According to cosmeticsinfo.org, the safety review of salicyclic acid had three issues for the chance of skin irritation, sun sensitivity and reproductive toxicity.

Sulfur

Sulfur is used in a variety of skin and hair care products and has been permitted by the FDA for these OTC applications. It is a natural element. It is a treatment for both acne and for dandruff. Sulfur in an anti-acne formula works by reducing the amount of pimples and blemishes in the skin. When used as an acne treatment ,it is generally in a three to 10 percent concentration. When mixed with other anti-acne medications, it is lessened to three to eight percent concentration.
